The Medicaid Application Center provides assistance in completing application for potential eligible person seeking health care coverage. Medicaid representatives are available to screen criteria for eligibility and process Medicaid applications.
What Is Medicaid?
Medicaid is a no cost health insurance program designed to assist eligible persons with coverage for medical care and related services.

Where Does A Person Apply?
You may apply at several Application Centers located in New Orleans Health Department Child Health Clinics, Monday thru Friday, from 8:00 a.m. to 3:00 p.m.
What Should I Bring?
- Proof of Income (earned and unearned)
- Social Security Card for each person applying
- Birth Certificate (if not born in Louisiana)
- Pregnancy statement from doctor (if currently pregnant)
- Health Insurance Card or Statement of benefits (if currently covered by private health insurance)
CHAMP
Child Health And Maternity Program that provides coverage for pregnant women and their children born after October of 1983.
LIF-C
Low Income Families with Children provides coverage for parents and children no longer eligible for AFDC.
PE
Presumptive Eligibility provides temporary coverage for pregnant women.
QMB
Qualified Medicare Beneficiary provides coverage for individuals receiving Medicare Part A.
SLMB
Specified Low Income Medicare Beneficiary provides coverage for individuals receiving Medicare Part A and B.
MNP
Medical Needy Program provides coverage for individuals whose income meets their basic needs, but is insufficient to meet medical expenses.
LTC
Long-Term Care provides coverage for residents of a skilled Nursing and Inter-mediate Care facilities.
What is LaCHIP?
LaCHIP (Louisiana Child Health Insurance Program) is an expansion of the existing Medicaid program that uses special income levels. It provides health coverage for uninsured children, from birth to 19 years of age. Even working families may qualify for this no cost health insurance coverage for their kids if income is at or below 200% of the Federal poverty level, $36,200.00 per year for a family of four as of April 1, 2002.

Do Recipients Have A Choice Of Where Services Are Received?
Yes, recipients are to contact the doctor of their choice to verify if the doctor or provider accepts Medicaid. If the doctor does not accept Medicaid or you do not currently have a doctor, call the toll free number 1-800-259-4444 for a referral.
