Certificate/Proclamation Request

Proclamation/Certificate Request Policy

The City of New Orleans continues to honor many local and national organizations, events, and individuals for outstanding humanitarian efforts by presenting them with an official proclamation from the Mayor of New Orleans.

Proclamations are only used for:

  • Organizational Honors
  • Church Anniversaries & Pastoral Anniversaries
  • Retirements
  • Heroic Deeds
  • 100th Birthday Celebration
  • Pastoral Anniversary
  • Outstanding Community Service (Individual, groups, & organizations)
All persons requesting proclamations and/or certificates must print or download the Proclamation Request Form below, or pick up a form at the receptionist desk located on the 2nd floor of City Hall in the Mayor’s Office.

Applications must be completed and submitted 4-5 business days in advance of event.

Upon approval, Proclamations and/or certificates can be picked up on Tuesdays or Thursdays from the Mayor’s Office, Suite 2E04, 1300 Perdido St., during business hours (8:30 a.m. – 5:00 p.m.).

Max. 10 proclamations are allowed per event.
